Use the following Checklist to collect your tax information. It will help you remember all important information relevant to your tax preparation. The list is comprehensive, however you only have to pick what is applicable to you. Click here to know where you can find the required information.

Personal Data

  1. Social Security Numbers and Date of Birth (including spouse and children)

  2. Child care provider : Name, address and tax I.D. or Social Security Number

  3. Alimony paid: Social Security Number

  4. If you are a new Client, please send -

    • Your ID like SSN Card/ Driver's License/ Passport Copy

    • Your spouse's ID like SSN Card/ Driver's License/ Passport Copy

    • Your Dependents Birth Certificate/Passport copy with I-94/School record/Immunization Certificate

    • Copy of last year Federal & State return (If filed elsewhere)

  5. If you are Filing NY & AL State Taxes, (Resident, Non Resident, Part year resident) please send us a copy of your & your spouse's Driver's License/State Issued ID. These can be issued by any State

  1. W-2 forms for this year

  2. Unemployment compensation: Forms 1099-G

  3. Miscellaneous income including rent: Forms 1099-MISC

  4. Partnership, S Corporation, & trust income: Schedules K-1

  5. Pensions and annuities: Forms 1099-R

  6. Social Security/RR1 benefits: Forms RRB-1099

  7. Alimony received

  8. Jury duty pay

  9. Gambling and lottery winnings

  10. Prizes and awards

  11. Scholarships and fellowships

  12. State and local income tax refunds: Form 1099-G

 

Homeowner/Renter Data

  1. Residential address(es) for this year

  2. Mortgage interest: Form 1098

  3. Sale of your home or other real estate: Form 1099-S

  4. Second mortgage interest paid

  5. Real estate taxes paid

  6. Rent paid during tax year

  7. Moving expenses

 

Financial Assets

  1. Interest income statements: Form 1099-INT & 1099-OID

  2. Dividend income statements: Form 1099-DIV

  3. Proceeds from broker transactions and Date of Purchase: Form 1099-B

  4. Retirement plan distribution: Form 1099-R

 

Financial Liabilities

  1. Auto loans and leases (account numbers and car value) if vehicle used for business

  2. Student loan interest paid

  3. Early withdrawal penalties on CDs and other time deposits

 

Automobiles

  1. Personal property tax information

 

Expenses

  1. Gifts to charity (qualified written statement from charity for any single donations of $250 or more)

  2. Unreimbursed expenses related to volunteer work

  3. Unreimbursed expenses related to your job (travel expenses, uniforms, union dues, subscriptions)

  4. Investment expenses

  5. Job-hunting expenses

  6. Job-related education expenses

  7. Child care expenses

  8. Medical Savings Accounts

  9. Adoption expenses

  10. Alimony paid

  11. Tax return preparation expenses and fees

  12. Education Expenses For Children For Dependents For Spouse

 

Self-employment Data

  1. Business income: Forms 1099-MISC and/or own records

  2. Partnership SE income: Schedules K-1

  3. Business-related expenses: Receipts, other documents & own records

  4. Farm-related expenses: Receipts, other documents & own records

  5. Employment taxes & other business taxes paid for current year: Payment records

 

Miscellaneous Tax Documents

  1. Federal, state & local estimated income tax paid for current year: Estimated tax vouchers, cancelled checks & other payment records

  2. IRA, Keogh and other retirement plan contributions: If self-employed, identify as for self or employees

  3. Records to document medical expenses

  4. Records to document casualty or theft losses

  5. Records for any other expenditures that may be deductible

  6. Records for any other revenue or sales of property that may be taxable or reportable
